Health care providers and community members are welcome to contact the Advance Care Planning Program at Cottage Health. The Advance Care Planning Team can help with the following:
Answering questions about completing Advance Directives and Physician Orders for Life-Sustaining Treatment (POLST) forms, in addition to answering legal questions.
Submitting a completed Advance Directive and/or POLST to an individual’s electronic medical record at Cottage Health.
Providing current Cottage Health inpatients with individual Advance Care Planning counseling at bedside during inpatient stay.
Referring community members to organizations which will help them complete their Advance Directives outside of the hospital.
Providing MyCare Advance Directives and MyCare informational brochures to health care providers, professionals, and community members in Santa Barbara County.
Presenting information about Advance Care Planning to health care providers and professionals in Santa Barbara County.

Sansum Clinic offers Advance Directive workshops, free of charge and open to the community. In addition to the workshop, patients of Sansum Clinic may get individual help with Advance Care Planning as well as free notary service for their Advance Directives.
