We partner with local medical clinics to offer appointments directly at their facilities. This eliminates barriers and allows us to use your medical visit to provide you with information and support about your Advance Directive in an easy and efficient way.

What to Expect in an Embedded Clinic Appointment
Where These Appointments Take Place
Personalized appointments are held at our offices, as we have access to a printer and additional staff who can act as witnesses if needed. If you are unable to come to our offices, we can also schedule an off-site appointment, depending on our facilitators' availability.
Home visits may be made due to transportation or mobility challenges.

How It Works
Our staff will coordinate with you and a trained ACP facilitator to schedule an appointment in one of our partner clinics.
The facilitator will walk you through the MyCare Advance Directive and answers any questions you may have. Appointments usually take 1 hour.
If you are ready to complete your Advance Directive, we will arrange a notary or witnesses to legalize the document and make copies to submit to your healthcare provider. If you need more time, our facilitator will help you schedule a second appointment.
What to Bring to the Appointment
If you are planning to complete your Advance Directive at the appointment, please bring a current Government ID such as a Driver’s License or Passport, along with any important documents you would like to refer to.
Loved ones are welcome to come for support or to complete their own Advance Directives.
