Get It Done SB offers several ways to help you complete your Advance Directive. Whether through community workshops, one-on-one support, or appointments at your medical clinic, we're here to guide you through the process. Scroll down to learn about the different ways we can help.
Community Workshops
Our workshops are one of the most effective ways to complete your document. They are offered regularly at different locations throughout South County so you can easily participate.

Our workshops are conducted in two parts. In the first week, we provide a detailed review of the document and answer your questions. In the second week, a notary is available to help you complete and formalize your document. You will also have the opportunity to work individually with a trained facilitator to ask questions and clarify any doubts.

You are 18 years of age or older.
You want to prevent your family from having to make difficult medical decisions.
You want to appoint a trusted person who can make medical decisions for you if you are unable to do so.
You want to understand and make informed decisions about: resuscitation, ventilator use, artificial nutrition, and organ donation.

Personalized assistance: Trained facilitators guide you step-by-step through completing your Advance Directive.
Free notary: If needed, we will arrange for a notary at no cost to you.
Integration with your medical records: We will send your document directly to your medical system so it is available whenever needed.
One-on-One Appointments
If you are unable to attend one of our workshops or need to complete your document more quickly, we offer personalized services for you and your family. We can schedule a one-on-one appointment where you will work with a trained facilitator who will help answer your questions and complete your Advance Directive.

Personalized appointments are held at our offices, as we have access to a printer and additional staff who can act as witnesses if needed. If you are unable to come to our offices, we can also schedule an off-site appointment, depending on our facilitators' availability.

Personalized assistance: Trained facilitators guide you step-by-step through completing your Advance Directive. These appointments are especially helpful if you have an upcoming surgery, if your family has questions or concerns about the conversation, or if you simply want to ask many questions and receive individual guidance.
All in one place: We provide everything you need to complete your Advance Directive: step-by-step guidance, personalized assistance, witnesses to legalize your document, final copies, and delivery to Cottage Hospital, ensuring your document is available when needed.

Our team will submit completed documents to Cottage Health to be entered into your medical record. We also provide you with copies to give to loved ones, physicians, and others.
Embedded Clinics
We partner with local medical clinics to offer appointments directly at their facilities. This eliminates barriers and allows us to use your medical visit to provide you with information and support about your Advance Directive in an easy and efficient way.

We've partnered with Santa Barbara Neighborhood Clinics (SBNC) to offer appointments at their facilities. All SBNC patients are welcome to schedule an appointment. We currently offer appointments at the Isla Vista, Goleta, and Eastside clinics, and we're working to add more locations soon.

If you are an SBNC patient, you can ask your doctor to make a referral for Advance Care Planning.
Otherwise, you can contact Diana Soto, SBNC Wellness Navigator, at 805-690-6528 ext. 2027, or call Mi Regalo and mention that you are an SBNC patient and prefer your appointment to be held at the clinic.

Our appointments at medical clinics combine all the benefits of our personalized services—step-by-step guidance, one-on-one assistance, witnesses to legalize your document, final copies, and delivery to Cottage Hospital—with greater convenience and efficiency. If you already have a scheduled medical appointment, we can use it to complete your Advance Directive and save you from having to visit another location.
In addition, if you have medical questions or a health condition, your medical team will be available to answer specific inquiries about your situation.
