Our Advance Care Planning Workshops

Opportunity to Get It Done! Join us at our community Advance Care Planning workshops to complete your Advance Directive for free!

  • It is an important legal form that helps your medical team and loved ones know and respect your health care decisions in the event you have a medical emergency or are seriously ill and cannot speak for yourself.

  • Any adult over 18 years of age. We invite you to extend the invitation to the person who you think may be your representative in health care matters.


    • Why is advance health care planning important?

    • Who can be your representative in health care matters and what is their role?

    • Options and explanation of life-prolonging treatments: cardiopulmonary resuscitation, ventilator, feeding tube, and dialysis machine

    • One-on-one help from experts who can answer any additional questions you have and who will help you complete the form in its entirety

Our One-on-One Advance Care Planning Services

Use our personalized services to complete your document now! We have trained and qualified volunteers to help you complete and legalize your Advance Directive (MyCare).

  • Trained facilitators are available to help all community members complete their MyCare Advance Directive via one-on-one appointments at their residence, our offices, or out in the community.

  • We will coordinate the use of a notary free of charge, in cases of financial need, to legalize the Advance Directive.

  • Our team will submit completed documents to Cottage Health to be entered into your medical record. We also provide you with copies to give to loved ones, physicians, and others.

Our Embedded Clinic Program with Santa Barbara Neighborhood Clinics

With our embedded clinic model, the Get It Done SB! team holds on-site office hours at Santa Barbara Neighborhood Clinics (SBNC) throughout the month. We're here to educate, guide, and support you through meaningful Advance Care Planning (ACP) conversations — right where you already receive your care.
