If you are unable to attend one of our workshops or need to complete your document more quickly, we offer personalized services for you and your family. We can schedule a one-on-one appointment where you will work with a trained facilitator who will help answer your questions and complete your Advance Directive.
What to Expect in a One-on-One Appointment
Where These Appointments Take Place
Personalized appointments are held at our offices, as we have access to a printer and additional staff who can act as witnesses if needed. If you are unable to come to our offices, we can also schedule an off-site appointment, depending on our facilitators' availability.
Home visits may be made due to transportation or mobility challenges.

How It Works
Our staff will coordinate with you and a trained ACP specialist to schedule an appointment in a private setting.
The facilitator will walk you through the MyCare Advance Directive and answers any questions you may have. Appointments usually take 1 hour.
If you are ready to complete your Advance Directive, we will arrange staff to witness the document and make copies to submit to your healthcare provider. If you need more time, our facilitator will help you schedule a second appointment.
What to Bring to the Appointment
If you are planning to complete your Advance Directive at the appointment, please bring a current Government ID such as a Driver’s License or Passport, along with any important documents you would like to refer to.
Loved ones are welcome to come for support or to complete their own Advance Directives.
